Coronary artery stents.

چکیده
CONTEXT Intracoronary stents are now used for the majority of patients undergoing percutaneous coronary revascularization, and the body of scientific knowledge about stents has expanded rapidly in the last several years. OBJECTIVE To review the evidence supporting the widespread use of intracoronary stents. DATA SOURCES The MEDLINE database was searched for articles from 1990 through January 2000 using the indexing terms stents, coronary artery disease, and angioplasty. Additional data sources included bibliographies of articles identified on MEDLINE, bibliographies in textbooks on percutaneous coronary interventions, and preliminary data presented at recent national and international cardiology conferences. STUDY SELECTION We selected for review studies that assessed the effects of stenting on the immediate and long-term outcome of patients undergoing percutaneous coronary revascularization. If data from randomized controlled trials were not available for specific patient subsets or lesion characteristics, observational studies were included. DATA EXTRACTION The methodologic characteristics of studies in coronary stenting were extracted and summarized according to key components of research design, including lesion type, location, and adjunctive therapy used. Studies were classified according to the strength of the available data into proven and unproven indications for stent use. DATA SYNTHESIS Coronary artery stents increase the safety of interventional procedures, increase procedure success rates, and decrease the need for emergency coronary artery bypass graft surgery. CONCLUSIONS Intracoronary stents have become an essential component of the catheter-based treatment of coronary artery disease. The evidence indicates that elective stenting, rather than provisional stenting or balloon angioplasty alone, improves clinical outcomes in the months following percutaneous coronary revascularization in a wide variety of clinical settings and lesion types. Optimal Stent Expansion by Stent Balloon Multiple Inflation at Nominal Pressure in Resistant Lesions: A New Technique
Introduction: This study aimed to evaluate the effects of four-time inflation of the stent balloon at nominal pressure on optimal stent expansion in resistant lesions. Materials and Methods: This interventional study was conducted on 39 patients with coronary artery lesions, in whom Zotarolimus-eluting stents (N=20), Paclitaxel-eluting stents (N=11) and other stents (N=8) were deployed four tim...

Background: This study assessed the application value of 64-slice spiral computed tomography angiography (CTA) in a follow-up evaluation of patients receiving coronary stent implantation. Materials and Methods: A total of 468 patients who underwent percutaneous coronary intervention (PCI) at our hospital between January 2013 and October 2016 were selected for this study. Coronary angiography an...
متن کاملOutcomes of coronary artery bypass grafting versus percutaneous coronary intervention with second-generation drug-eluting stents for patients with multivessel and unprotected left main coronary artery disease
OBJECTIVES To compare the efficacy and safety of percutaneous coronary intervention using second-generation drug-eluting stents with those of coronary artery bypass grafting among patients with multivessel disease and/or unprotected left main coronary artery disease in terms of mortality, myocardial infarction, repeat revascularization, and angina. BACKGROUND Although coronary artery disease ...
متن کاملMultiple giant coronary arterial aneurysms following sirolimus drug eluting stents implantation.
Coronary artery aneurysm after coronary interventions are rare but variable (0.2%-1.7%) clinicopathological entity depending on whether a incidental or a routine 6 monthly angiographic follow up finding and is seen more common with drug eluting stents (DES) rather than bare metal stents (BMS). It is of three types and depending on the type and patient clinical response further strategy (conserv...
متن کاملCoronary artery bypass grafting following in-stent restenosis of drug-eluting stents deployed in the left main coronary artery.
Two cases of drug-eluting stent restenosis after percutaneous coronary intervention in the left main coronary artery and its bifurcation are presented. An off-pump coronary artery bypass grafting following in-stent restenosis was performed. Drug-eluting stents have shown a reduced frequency of in-stent restenosis and a good safety profile compared with bare metal stents. However, intervention w...
